Wellness Center

The Wellness Center is a 21,500 square-feet facility housing the Title VI Elders Program, an exercise facility, walking track and the Diabetic Initiative Program. The exercise facility staff work with individuals to customize exercise programs that best fit their physical needs.

Diabetes Program

The Diabetes Program is accredited by the American Diabetes Association and is funded through Special Diabetes Programs for Indians (SDPI) Grant. The diabetes team is comprised of certified diabetes care and education specialists, registered dietitians, registered nurses and administrative staff. It serves individuals newly or previously diagnosed with diabetes in varying stages of life, through a variety of individual and group sessions with the mission to empower individuals to manage their diabetes and prevent associated complications by promoting healthy living.

Behavioral Health

Behavioral health services are provided to clinic patients referred by their primary care providers or self-referred. Your initial contact with the receptionist will include completing forms to establish a separate behavioral health chart in the clinic. A therapist will be assigned, and an appointment will be scheduled.

WIC

Citizen Potawatomi Nation’s Women, Infants, and Children program provides supplemental nutritious foods, health and nutrition education, referrals, and nutrition and breastfeeding counseling to eligible pregnant, breastfeeding, and postpartum women as well as infants and children age 5 and younger. The program is designed to positively impact prenatal nutrition, infant birth weight, iron deficiency anemia, and early childhood nutrition and cognitive development. WIC complies with all applicable federal and state agency mandates.
